Aloha Activation Network Associate Organization Application

A recognized partner with Aloha Activation Network (AAN).

Terms:

1. All donations collected on your behalf are processed free of charge unless any unusual fees are required. (We will not process donations if there are conditions attached.)

2. We recommend you tithe 10% of what you receive on all donations collected by your organization through AAN. 

3. We would appreciate it if you would display AlohaActivationNetwork.com with the logo on your website

Requirements:

2. Read, understand, and approve the AAN mission: AlohaActivationNetwork.com

3. Sponsored by an AAN officer

4. Sign the Contract/Agreement

Partner Request:

Email sent to Partners@AlohaActivationNetwork.com       

Please put "Associate Organization Application" in the email subject and include the following:

1. Name: Your full name as filed with the IRS 

2. Doing Business Names (DBA)

3. Your logo to be next to your name on our website

4. Mailing address: to show proof of residency

5. Email address: contact info

6. Textphone number: cell number for quick info/contact

7. In the body of the email, please include

   1. A brief statement of what you believe the mission of AAN is

   2. Executive officer sponsor: _______________________. If you need a sponsor, please let us know

   3. Any questions you might have.

   4. At the bottom of the email before typing your name write the following statement: "I accept the terms and acknowledge that all the information provided above is true and accurate to the best of my knowledge."

   5. Note: further information may be requested by your sponsor.
